Clients Under 18

Clients who are under the age of 18 are welcome to receive services; however, a parent or legal guardian must complete and sign all required consent and waiver forms prior to the appointment. These forms can be sent electronically via email or completed in person if the parent or guardian chooses to accompany the client.

During checkout, you will be asked to read the forms, provide your age and indicate how your parent or guardian would like to receive the intake forms.

Please note, a parent or guardian is not required to be present during the service itself, but all consent forms must be signed in advance. For verification purposes, any client who appears to be under the age of 18 will be asked to provide a valid form of identification.
